Meibomian Gland Dysfunction - More Than Just Dry Eye

Meibomian gland dysfunction (MGD) is one of the most common causes of dry eye disease. The eyelids contain specialized oil glands known as the meibomian glands, which release an oil called meibum. Similar to how sebaceous glands in the skin release sebum to maintain hydration and protect the skin barrier, meibom helps stabilize the tear film and prevents tears from evaporating too quickly. This protective oil layer is essential for maintaining comfortable, healthy vision and reducing irritation and dryness.

Over time, a variety of factors can negatively affect these glands, including natural anatomical changes, environmental exposure such as dust or allergens, excessive screen time, certain medications, and systemic health conditions. These factors can lead to chronic inflammation and obstruction of the glands, causing the oil to become thickened and trapped. When this process persists for months to years, the glands may begin to degenerate, resulting in permanent gland loss. This is concerning because once significant gland atrophy occurs, the glands may no longer adequately produce or release oil, leading to worsening dryness, irritation, fluctuating vision, and chronic ocular surface disease.

Fortunately, newer treatment technologies such as IPL (Intense Pulsed Light), RF (Radiofrequency), and LLLT (Low Level Light Therapy) have shown promising results in helping improve meibomian gland function. These treatments can help reduce inflammation, soften and release obstructed oils, and improve gland expression by opening clogged glands. Studies have also suggested that early intervention with these therapies may help stimulate gland recovery and preserve gland structure before permanent loss occurs.
